You Know You Are A Candidate For LASIK - Now What?

There are many resources available to explain the LASIK procedure; however, very few detail the testing necessary leading up to the procedure. These tests can be seemingly overwhelming and technical. Some of the testing could have been done at an initial consultation, though, some tests may need to be repeated. Each test has a specific purpose to assist the surgeon in obtaining the best results possible.

The first step is to determine the uncorrected visual acuity, which is vision without glasses or corrective lenses. This raises the question of “What does 20/20 mean?” In the U.S. vision is recorded on a chart with standardized letters calibrated to be read at 20 feet away. The fraction that describes vision, for instance, 20/20, 20/40, compares the smallest letters that a person with perfect vision can see at 20 feet to your vision at 20 feet. If you are only capable of seeing larger letters on the chart that can be seen by a normal person standing back 40 feet from it, then your vision is 20/40. 20/400, means at twenty feet from the chart you can only distinguish the giant E on top, which could be seen by a normal person from 400 feet away.

Some visual acuity levels bear special legal significance. For example, you must be able to see the 20/40 line of letters, or smaller, in order to drive without glasses in the U.S. For this reason, statistics for laser vision correction are often quoted in relation to the percent of patients receiving 20/40 vision or better. If the best vision a person can see with corrective lenses is 20/200 in both eyes, that person is legally blind.

Next, best-corrected visual acuity must be determined. Refractive error or acuity is measured one eye at a time, covering the other eye with an occluder. Refractive error is the state of the eye for which a prescription is normally given for glasses or contacts. Generally two refractions are completed on a person considering LASIK surgery. The first refraction is called a manifest refraction. A determination of the refractive error is the “dry” or undilated state. The second is a cycloplegic refraction. This is similar to a manifest refraction except it is completed while your eyes are dilated. The reason for the cycloplegic refraction is to insure that the manifest is correct. When your eyes are dilated, you are unable to accommodate (focus up close) during the test.

Once your visual acuity has been determined, the results will be compared to your previous prescription to assure stability in refractive error. There should not be more than a half a diopter change in the past two years.

The next eye examination is performed with an instrument known as a slit lamp microscope that allows a binocular, high magnification view of the eye. This examination is used to rule out the presence of disease.

Keratometry is used to measure corneal curvature and is completed with a keratometer. This measures the shape of your cornea. Many surgeons have sophisticated topography equipment that obtains these measurements.

An Opthalmoscope is used to examine the back of the eye or “retina” to exclude any tears, detachment or predisposing conditions. The optic nerve that carries vision from the retina to the brain is also observed for signs of glaucoma.

Corneal Topography is used to demonstrate astigmatism and other abnormalities such as keratoconus. Topography is a map of the eye showing the hills and valleys in color. Today’s more progressive equipment also measures pachymetry and keratometry during the course of the test.

Pachymetry is a measurement of the thickness of the center of the cornea. An ultrasound probe touching the surface of the cornea after an eye drop is administered for anesthesia constitutes Pachymetry. The thickness of the cornea is essential to determine the amount of correction that can be obtained. When LASIK is performed the laser ablates or removes some of the corneal tissue. The more correction that is needed, the more tissue is removed. This test along with the refraction is used to make sure there will be enough tissue to maintain a stable healthy cornea.

Determination of the dominant eye is obtained for those patients who are either having one eye completed at a time or they have chosen to have monovision. If a person is having one eye completed at a time, most surgeons begin with the dominant eye.

The final test is the measurement of pupil size. This is important for those people who have very large pupils in dim light. This information can assist the surgeon in determining the size of the ablation area to avoid an extreme amount of glare and starburst post surgically in night driving.

All of these tests are necessary to have the most accurate results possible with LASIK surgery; nevertheless, it is always best to consult with an ophthalmologist with regards to specific circumstances.

Newsletters Can Help Your Small Business Keep Customers

One key to keeping clients coming back for more is to make sure that they don’t forget about your small business and the services it provides. This doesn’t mean barraging customers with junk mail or spam, but by balancing your marketing efforts to provide value. One great way to do this is with newsletters.

Newsletters can be electronic or paper or a combination of both.

Print Newsletters
For established customers, a quarterly or monthly newsletter is a great way to keep you business fresh in their minds while providing valuable information related to your area of expertise. For example, my investment advisor sends out a quarterly, four-page traditional type newsletter filled with great tips that are relevant to the time of year. I love getting it because the information is great; and for him, well he’s my go to guy when it comes to financial questions. His newsletter is not only establishing him as an expert, but also helping to build the relationship with me, his client.

The newsletter can be as large or as small as you want, the important thing is to provide valuable, relevant information that your customers will want to receive. You can write the pieces yourself, or there are several resources online for free reprint articles on tons of subjects. You can also hire someone to write the pieces for you - it’s more affordable than you may realize and unlike reprint pieces, these will be attributed to you (no references to other businesses with the same expertise).

Electronic Newsletters
An electronic newsletter is similar to a traditional one in content, however, there are considerations. CAN-Spam laws need to be taken into consideration when sending the newsletter as do delivery options. There are many services available that will help to make sure your newsletter is compliant with anti-spam laws as well as manage your list and delivery options and I strongly recommend using one.

I don’t recommend sending more than once a month unless you have valuable information your customers will want to know. Also remember, people get LOTS of email, so over-sending is one way to ensure your e-mails will end up in the trash or your subscribers will UN-subscribe.

Which is Better: Print or Electronic?
The answer to this question is dependent on your market. In some cases a combination of the two can work affectively as well. You will have to determine what your customer demographics are and which option would be most effective for your small business.

My mantra is “Communication, communication, communication,” and newsletters are just one more way to get your message to your customers. By sharing your valuable knowledge with your customers you are reinforcing the value of your small business and supporting the relationship you have with your customers. I encourage you to take the first step and start thinking about ways a newsletter can work for your small business.

Attention Deficit Disorder tends to focus predominately on children, leaving the ADD adult largely under served. Most of the information presented about Attention Deficit Disorder focuses on children, parenting and school issues. First, it is important that a physician rule out conditions like anxiety, depression, hypothyroidism, manic-depressions or obsessive compulsive disorder that can mimic Attention Deficit Disorder symptoms. Women should rule out perimenopause if the Attention Deficit symptoms appear in their late 30s or 40s.

If you need to finish a large project in one week, for instance, schedule specific time each day to work on a specific aspect of the project.

No Bubble-Bursting in 2006

No bubble-bursting is in sight for real estate sales in this new year of 2006. This is now expected to be the second best year in history for residential property sales, according to analysts at the National Association of Realtors. “Home sales are coming down from the mountain peak, but they will level-out at a high plateau - a plateau that is higher than previous peaks in the housing cycle,” said David Lereah, NAR’s chief economist. “This transition to a more normal and balanced market is a good thing.”

Even though mortgage rates have edged downward in recent weeks, they will generally trend upward during the year, probably to about 6.6 percent for a 30-year, fixed-rate mortgage, NAR predicts. Existing home sales, expected to reach about 7.1 million units in 2005 (when final figures are available), will probably decline a bit in 2006 - perhaps by about 3.7 percent to a volume of 6.84 million units. New home sales will be about 1.29 million units in 2005 and will probably drop by 4.8 percent to 1.23 million units this year. That would make this year the second best on record for new home sales.

“The housing market still is fundamentally healthy,” said Dave Wilson, president of the National Association of Home Builders. “Many builders sense some tapering off of buyer demand because of resistance to high prices and rising interest rates, and many companies have begun offering certain incentives in order to maintain their sales and production.” Confidence of home builders during December slid from its summer peak, yet remained well within the positive range, according to NAHB.

Thomas Stevens, NAR president, made this comment: “Housing has always been the soundest investment for most families. As the old saying goes, homeownership beats the heck out of a drawer full of rent receipts.

Thanks to our long experience in the market we have learnt to build trust with the client and when we are able to reply to each question, I wouldn’t say it is difficult. If you win a client who has four good friends, and if you reply the questions of the first one, the others say directly which property they want. Each detail has to be specified in advance. The information itself is extremely important. Everything should be clear and no surprises to rise forward.

How far Bulgaria is competitive with the quality and prices it offers?
- I definitely think that Bulgaria is quite competitive when there are so many clients that buy a property. If we are talking about the prices in Turkey, Cyprus and Spain, they are quite higher than Bulgarian ones. In the exhibitions we participate, we notice that the bigger part of the Turkish resorts are more expensive than Bulgarian ones, among which there are some non-furnished. And we offer furnished apartments complex with additional services, which the owner can use, and this is the way we win clients. This practice exists in other countries as well, but with the prices we sell, we turn into an attraction.

What should be added to the realized projects, in order to become more competitive?

- The infrastructure should develop further. It is obligatory to develop airports and golf-terrains. Both are foresee in Bansko, as well as at the Black Sea. There should be built a great service level and everybody have to work over it.
