You Know You Are A Candidate For LASIK - Now What?

There are many resources available to explain the LASIK procedure; however, very few detail the testing necessary leading up to the procedure. These tests can be seemingly overwhelming and technical. Some of the testing could have been done at an initial consultation, though, some tests may need to be repeated. Each test has a specific purpose to assist the surgeon in obtaining the best results possible.

The first step is to determine the uncorrected visual acuity, which is vision without glasses or corrective lenses. This raises the question of “What does 20/20 mean?” In the U.S. vision is recorded on a chart with standardized letters calibrated to be read at 20 feet away. The fraction that describes vision, for instance, 20/20, 20/40, compares the smallest letters that a person with perfect vision can see at 20 feet to your vision at 20 feet. If you are only capable of seeing larger letters on the chart that can be seen by a normal person standing back 40 feet from it, then your vision is 20/40. 20/400, means at twenty feet from the chart you can only distinguish the giant E on top, which could be seen by a normal person from 400 feet away.

Some visual acuity levels bear special legal significance. For example, you must be able to see the 20/40 line of letters, or smaller, in order to drive without glasses in the U.S. For this reason, statistics for laser vision correction are often quoted in relation to the percent of patients receiving 20/40 vision or better. If the best vision a person can see with corrective lenses is 20/200 in both eyes, that person is legally blind.

Next, best-corrected visual acuity must be determined. Refractive error or acuity is measured one eye at a time, covering the other eye with an occluder. Refractive error is the state of the eye for which a prescription is normally given for glasses or contacts. Generally two refractions are completed on a person considering LASIK surgery. The first refraction is called a manifest refraction. A determination of the refractive error is the “dry” or undilated state. The second is a cycloplegic refraction. This is similar to a manifest refraction except it is completed while your eyes are dilated. The reason for the cycloplegic refraction is to insure that the manifest is correct. When your eyes are dilated, you are unable to accommodate (focus up close) during the test.

Once your visual acuity has been determined, the results will be compared to your previous prescription to assure stability in refractive error. There should not be more than a half a diopter change in the past two years.

The next eye examination is performed with an instrument known as a slit lamp microscope that allows a binocular, high magnification view of the eye. This examination is used to rule out the presence of disease.

Keratometry is used to measure corneal curvature and is completed with a keratometer. This measures the shape of your cornea. Many surgeons have sophisticated topography equipment that obtains these measurements.

An Opthalmoscope is used to examine the back of the eye or “retina” to exclude any tears, detachment or predisposing conditions. The optic nerve that carries vision from the retina to the brain is also observed for signs of glaucoma.

Corneal Topography is used to demonstrate astigmatism and other abnormalities such as keratoconus. Topography is a map of the eye showing the hills and valleys in color. Today’s more progressive equipment also measures pachymetry and keratometry during the course of the test.

Pachymetry is a measurement of the thickness of the center of the cornea. An ultrasound probe touching the surface of the cornea after an eye drop is administered for anesthesia constitutes Pachymetry. The thickness of the cornea is essential to determine the amount of correction that can be obtained. When LASIK is performed the laser ablates or removes some of the corneal tissue. The more correction that is needed, the more tissue is removed. This test along with the refraction is used to make sure there will be enough tissue to maintain a stable healthy cornea.

Determination of the dominant eye is obtained for those patients who are either having one eye completed at a time or they have chosen to have monovision. If a person is having one eye completed at a time, most surgeons begin with the dominant eye.

The final test is the measurement of pupil size. This is important for those people who have very large pupils in dim light. This information can assist the surgeon in determining the size of the ablation area to avoid an extreme amount of glare and starburst post surgically in night driving.

All of these tests are necessary to have the most accurate results possible with LASIK surgery; nevertheless, it is always best to consult with an ophthalmologist with regards to specific circumstances.

Attention Deficit Disorder tends to focus predominately on children, leaving the ADD adult largely under served. Most of the information presented about Attention Deficit Disorder focuses on children, parenting and school issues. If you need to finish a large project in one week, for instance, schedule specific time each day to work on a specific aspect of the project.

How to Handle Mouth Ulcers

Recurrent mouth ulcers are a source of worry for many people worldwide. It is an apparently simple condition that causes much more pain than is expected. Because of its relative commonness in some societies, it has gotten itself a variety of names such as canker sore, aphthous stomatitis or aphthous ulcer. Commonly, mouth ulcer, canker sore or whatever name you wish to call it, involves painful open sores that will exist in the mouth, believed to be due to the breaking off of the overlying mucous membrane.

In most reported cases, mouth ulcers starts off with a sort of tingling or burning sensation at the spot where the future mouth ulcer will occur. After a few days, a blister forms in the mouth with the top quickly sloughed off resulting in a red spot or a bump, which will then be followed by an open ulcer. The mouth ulcer appears as a white or yellow oval with an inflamed red border. Sometimes a white circle or halo around the lesion can be seen. The grey, white, or yellow colored area within the red boundary is due to the formation of layers of fibrin around the ulcer.

When at this final stage, a mouth ulcer, in common cases can be as wide as 3mm, while in extreme cases it could be as large as 1cm (in extremely rare cases, though). The main ulcer body can be really painful when agitated especially with the presence of food in the mouth. This is perhaps the most unacceptable part of the disease. Also, mouth sores are often accompanied by painful swelling of the lymph nodes below the jaw and this fact tends to make people confuse canker sores for toothache, initially.

Several reasons and factors have been cited as the cause of mouth ulcers, though none seems to be conclusive yet. Some evidence points to the role of infections like herpes, cytomegalovirus and yeast as the cause of mouth ulcers. However, several other viruses that have not even been conclusively identified are also being researched for their role in the cause of mouth ulcers.

Moreover, a person’s immune system seems to play an important role in the cause of mouth ulcers, in some people. This process is known as an autoimmune reaction, in which in the response to certain allergies, such as to chemicals in toothpastes and/or mouthwashes, a person’s immune system causes the formation of blisters in the mouth which then open up to result in ulcers. Although, no studies have been able to isolate bacteria or viruses enough to establish definite roles in mouth ulcers, some studies have shown a rise in a person’s immunity against herpes and cytomegalovirus when that person suffers mouth ulcers.

Mouth ulcers may last a couple of days or in some cases, up to about 4 weeks, with the pain and discomfort persisting throughout. They may disappear without treatment, though this rarely occurs.

There are several ways to prevent and manage mouth ulcers. However, the best prevention of the disease unarguably requires sound dental hygiene which eliminates the presence of harmful microbes in the mouth, and to avoid/reduce spicy, acidic or salty foods and drinks, which tend to predispose to it.

Medically, there are only a few treatment options available to choose from. In most cases, your doctor will have to inject a small amount of a type of cortisone called triamcinolone into each ulcer. This will get rid of the ulcers rather quickly, but it will not prevent them from recurring. However, the best home remedy you can employ is to pour salt on the surface of the main ulcer. This could be really painful though, but it relieves you of the continuous, daily pain that accompanies mouth ulcers.

The US Food and Drug Administration has approved the use of an oral paste known as amlexanox (Aphthasol). When applied, this paste reduces the swelling and discomfort of mouth ulcers. Moreover, clinical trials reported in recent journals, have established the efficacy of several treatment options for mouth ulcers. Some of these include the use of prednisone to suppress immunity (reducing autoimmune responses that are believed to cause mouth ulcers in some people), pentoxifylline, a drug that increases blood flow to the mouth, thalidomide to reduce blood flow to the area affected by the ulcer, acyclovir to kill herpes, and colchicine, a gout medication.

As you can see, there are several options available when you really need to battle that troublesome sore in your mouth. You only need to choose the option most suited to you or contact your physician immediately you feel you have a canker sore or mouth ulcer, for advice.

The Right Pillow for a Stomach Sleeper

I’ve slept on my stomach ever since I can remember. Even though I have heard that many people advise that sleeping on your stomach isn’t the best position, it just seemed to work for me. At one time I tried to change my sleeping position but I just ended up frustrated and loosing sleep time. However, I was starting to notice that even though my body was used to sleeping on my stomach, I would wake up with a sore neck and back.

I didn’t pay much attention to these symptoms, I thought it was normal. That is until I realized that my stomach sleeping habit was really affecting my mood since I would always wake up grouchy and with neck pain. At first I wasn’t sure what the problem was and I knew that having to try to change my sleeping position (again) would be a complete failure because I knew I was just stuck as a stomach sleeper.

Diet For Ulcers

At present there is no specific dietary treatment to cure a peptic ulcer. Mainstream medical opinion considers that drug therapy offers the most cost-effective treatment for ulcers, with surgery as an option in only the most severe cases. Even so, studies show an association between certain foods and the risk of developing ulcers, and doctors continue to warn that unbalanced diets can make symptoms worse. A permanent cure for ulcers may yet involve a combination of medication and diet.

What Is An Ulcer?

An ulcer is a small sore or break in the lining of the stomach or just beyond the stomach in the first part of the intestine, called the duodenum. Once the break occurs it is gradually eroded and deepened by the corrosive effect of acidic gastric juices to form an ulcer. An ulcerous condition may develop quickly or over a long period of time. An estimated 5-10 percent of people suffer from an ulcer at some point in their lives. Ulcers affect people of all ages and are equally common in men and women.

Difference Between Duodenal and Stomach Ulcers

A duodenal ulcer is located in the duodenum. This type of ulcer is about three times more prevalent than a gastric ulcer which forms in the stomach. Collectively, they are known as peptic ulcers.

What Causes a Peptic Ulcer?

An ulcer develops when the balance between the acidic digestive juices and the protective lining of the stomach and intestines (mucous membrane)is disturbed. This leads to a corrosion of the lining, permitting ulcers to form. Experts now understand that a common cause of developing peptic ulcers is a bacterium (a germ) called Helicobacter pylori.

Researchers estimate that up to 70 percent of the world’s population is infected with H.pylori. The bacteria lives in the lining of the stomach, and is found in virtually all duodenal ulcers and 8 out of 10 gastric ulcers.

Another cause of ulcers is the use of non-steroidal anti-inflammatory drugs (NSAIDs), such as aspirin.

What Are The Symptoms Of An Ulcer?

Most ulcers cause recurring episodes of abdominal pain, sometimes radiating to the back. These episodes may be separated by periods of remission lasting from several weeks to years. The pain, often causing a burning sensation, is typically reduced by food but returns when the stomach empties. Pain experienced during the night is a common indicator of a duodenal ulcer. Other symptoms may include indigestion, nausea, vomiting, weight loss and black tarry stools. In a very small percentage of cases, the ulcer may rupture a blood vessel, causing vomiting of blood. Peptic ulcer disease can be very distressing and, if left untreated, can cause premature death in extreme cases.

How Is An Ulcer Diagnosed and Treated?

Diagnosis of an ulcer is typically performed either by endoscopy (a viewing tube passed down the throat into the stomach) and tissue sample (biopsy), or by giving the patient a barium meal and then an X-ray examination. Helicobacter pylori bacteria can be diagnosed by endoscopy, or a relatively simple “urea breath test”, or by a blood test.

Where the ulcer is related to H.pylori infection, a permanent cure can be achieved through medication alone. A stomach-acid reducing drug combined with antibiotics usually eradicates the bacteria and either cures the ulcer in a matter of weeks or leads to long term remission. Where the ulcer is unrelated to bacteria, stomach-acid reducing medication may be prescribed on an ongoing basis to protect the stomach lining. Although the majority of peptic ulcers respond well to such treatment, many patients suffer a relapse within 12 months. Thus the search for a permanent cure continues.

Diet And Ulcers

In general, if you want to ease the symptoms of an ulcer, or reduce your risk of developing one in the first place, you should avoid eating large meals, as they encourage the production of excessive amounts of stomach acid to digest the food. In addition, diets which are high in fat and sugar, but low in fiber are known to cause a variety of adverse effects in the gut. Here are some specific dietary tips which may help to reduce the symptoms and formation of peptic ulcers.

Reduce Your Intake of Spicy Foods

Although there is no clear evidence to suggest these foods cause ulcers, they have been shown to aggravate symptoms. So avoid foods like: chili peppers, black pepper, mustard, curry and other strong spices.

Reduce Alcohol And Caffeine Intake

Both alcohol and caffeine can be problem foods for anyone with an ulcer, or at risk from an ulcer, and should be avoided or consumed sparingly. These items typically increase the acidity of the stomach and therefore can contribute to ulcer development or aggravate symptoms of existing ulcers.

Reduce Your Intake Of Sodium

An American study has linked high intakes of salt and soy sauce to a higher risk of stomach ulcers. So avoid adding salt to your the food on your plate and use food labels to check for low-sodium varieties. Foods commonly high in sodium include: canned soup, tortilla chips, potato/corn chips, salted nuts, salted meats (eg. bacon), blue cheese, cornflakes. Switching to a diet containing fewer packaged or processed foods, will definitely reduce your sodium intake.

Add Oily Fish To Your Diet

Some studies suggest that omega-3 fatty acids - found in oily fish, like salmon, mackerel, sardines and herring - may help reduce the risk of ulcers. It seems that these omega-3 fats increase our production of prostaglandins, compounds that help to protect the lining of the alimentary canal, that is, the entire digestive passage, including the lining of the stomach and intestines.

General Advice About Dietary Vitamins And Minerals

The protective antioxidant effects of certain micronutrients should help to lower the risk of ulcers and ease symptoms when an ulcer has already formed. For instance, some nutritionists recommend eating more vegetables and fruit, such as carrots, kale, red/green peppers, cabbage juice, apricots and kiwi fruit, for their beta-carotene and vitamin C content, in order to help protect the lining of the stomach and intestine. Vitamin E from foods like wheatgerm, hazelnuts, cold-pressed sunflower seed oil, soybean oil, should also help, as should zinc from seafood and whole grains.

Other nutritionists are strong advocates of the healing properties of amino acids, the basic building blocks of proteins, and recommend L-Glutamine for peptic ulcers. Good food sources include: wheatgerm, cheddar cheese, almonds, sunflower seeds and sesame seeds.

Possible Herbal Remedies For Ulcers

In some tests, a tablet form of licorice root - which has been treated to remove an acid which may raise blood pressure - has been shown to protect against ulcers caused by the adverse effects of aspirin on the stomach lining. Camomile tea may also ease the symptoms of ulcers.

Lifestyle And Risk Of Ulcers

Damage caused by smoking increases the risk of ulcer formation and slows ulcer healing. Another reason to quit! The effects of stress on ulcer development are less easy to assess, although a stressful lifestyle clearly impacts on eating habits, alcohol consumption and smoking. Those Four Little Words, “You Need Reading Glasses”

It may not be as sure as death and taxes, but most of us aging baby boomers will be hearing those four little words, “you need reading glasses” from your eye care physician. What has happened to your perfect vision? Even if you were used to wearing glasses, it still can happen. This natural loss of vision making it difficult to read is called presbyopia. It may be time for bifocals if you already have a vision problem, but what if you have never worn glasses and all of a sudden you find yourself with arms too short? Reading glasses are the solution for you. You may have seen some in the drug stores, you know, those funny little glasses hanging next to the counter. They have different magnifications or plus powers that allow your eye to see close up again. If you are new to the eyeglasses world, you might want to be careful the first time you put them on. They can make you a little dizzy because most of the world out there will be blurry. They are just as the name implies; glasses for reading. Some people have problems walking and definitely don’t drive in them. You should also know that reading glasses characteristically focus in on a specific thing. When you look at a paper in front of you, you won’t be able to see clearly twelve to sixteen inches from the paper. This can also be tricky to adjust to. If this really is a problem for you, talk to your eye care physician about other things you can do.

Presbyopia can actually start occurring at age ten but usually doesn’t affect people until around forty. This is one of the fastest growing eye problems affecting us because the baby boomers are aging. As this demographic continues to age, you will see more people with reading glasses than ever before. You can’t prevent this problem and once you have it, it won’t go away. How do you know if you have this problem? Adult acne can be treated

Certain medications can also cause adult acne. Anabolic steroids are one medication which is responsible. Many of these types of steroids are used to bulk up and are also illegal. There are other legal medications which can also cause adult acne. Anti-epileptic medications, Lithium, anti-Tuberculosis medications, and medications containing iodine can all cause adult acne. Some of these medications can be discontinued or changed to prevent the acne, however, some cannot.

Learn To Recognize The Warning Signs of Hypothyroidism

The thyroid gland does not require extra attention or
maintenance most of the time. But when it acts up, you will
notice a change in your health marked by relevant symptoms.
Although these may be vague enough to avoid your physician’s
diagnosis for a time, with the right blood test and a careful
medical examination, you can soon find out if you have
hypothyroidism.

One of the first reasons to suspect that your thyroid may be
acting up is if you feel extra sluggish or worn out. Some people
report they get so tired and listless, that it becomes
increasingly difficult to get out of bed most days. They can
barely drag across the floor to get to the bathroom, and a
flight of stairs seems to ask the impossible. Of course, if you
get to this point, you need to see a doctor pronto, as your
thyroid may be in serious trouble. But be sure to ask your
doctor to evaluate any type of unexplained fatigue.

Another symptom is dry skin and hair. You may find that your
hair seems thin or brittle, and it could lose its sheen. Your
skin may become rough and flaky, no matter how much you try to
moisturize it. Like hair, skin can lose its natural vibrancy,
tone, and color, especially around the joints like your elbows.
Your mouth may feel drier than usual, as well.

Internal organs may not work properly and show signs of
problems. For example, you could develop frequent or recurrent
constipation. Your periods (if you are a female) could start
coming irregularly and may not have the same type of monthly
flow that you are used to. Your heart or breathing may not seem
to work as efficiently as before. This observation fits with the
fatigue mentioned above.

Since many of these symptoms are fairly generic, you will need
to see your doctor to get an examination and accurate diagnosis.
Your doctor may order blood tests to check the level of your
thyroid’s function. You might even be asked to take a
radioactive pill or receive an injection that will let a
technician check your thyroid’s activity level the next day or
so. Be sure to follow directions and keep pets or children away
for a day or two afterward until the radioactive matter passes
from your body.

If the doctor makes a diagnosis of hypothyroidism, you will
probably be given a prescription medication to supplement the
work your thyroid is supposed to do. Most of the time this
condition is permanent, and it could be serious or even fatal if
you don’t take the medication. That is why it is important to
check with your doctor about any problems you may be having so
you can get a speedy and correct diagnosis and treatment. If you
have the opposite kind of symptoms, such as a racing heart rate,
oily skin, and diarrhea, you could have hyperthyroidism or
another condition. Always let your doctor know if you experience
a significant change in bodily behavior.

Nothing is more important to our health than drinking lots of pure water. All of the complex biochemical functions in our bodies depend on this key substance.
